Form Center creates public forms for VPS service requests. A client opens a direct link or an embedded form, and each valid submission creates a New Service request with the Form source in Requests. Admin and Staff users can view forms, copy form links, open forms, get embed code, and view submissions. Only an Admin can create, edit, activate, deactivate, or delete a form, or change Form Settings.

Create a form

Open Form Center under Integrations in the sidebar, then select Create Form. Use Next to move through the builder.

Name the form (Setup)

Enter a unique Form Name. The panel identifies the form by this name and creates the form path from it. Description (Optional) appears on the public form and helps your team identify its purpose.Select an Approval Settings value:
  • Manual Approval keeps each submission pending until a broker user approves it. This is the recommended setting.
  • Auto Approval starts VPS provisioning immediately after submission.
Use an Auto Approval form internally only. Do not publish it for public use. A valid submission starts VPS provisioning without a broker review.

Configure fields and appearance (Design)

Fields sets the questions the client answers, and Appearance sets colors, logo, and layout. The defaults produce a working form, so change these only when you need to. See the builder reference for each control.

Set the result (Success Page)

Set the Success Message shown after submission. Optionally add a Redirect URL (Optional) to send the client to your own page after success. A redirect URL must use HTTP or HTTPS.

Add translations and recipients

Translations adds localized public content, and Notifications adds email addresses that receive submission alerts. Both steps are optional.

Create the form

Select Create Form on the Notifications step. The form is Active immediately, and the Complete step shows the Direct Link and Embed Code controls. See Publish and Embed Forms for sharing and slug requirements.
Select Previous, or select a completed step in the step list, to return to an earlier step. Select Cancel to close the builder without saving.

Form builder Fields step with standard field controlsEvery form keeps these required standard fields: Name, Email, Requested Plan, VPS Language, and Terms and Conditions. A new form also includes the optional Trading Account Number field, which you can remove. Use + Add Standard Field to add an optional Phone, Trading Account Number, or Additional Notes field that is not on the form.Use the drag handle to reorder fields. A standard field’s edit menu changes its Label, Placeholder, and Help Text. Select Save Changes to keep field edits.To limit plan choices, open the edit menu for Requested Plan, select Edit, and change Visible Plan Options. At least one plan option must remain selected.For Terms and Conditions, set Content Type to External Link (uses Terms and Conditions URL) or Inline Content (Markdown) (uses Terms and Conditions Content).To add a custom field, select Add Custom Field, choose a Field Type, enter a Label, complete the remaining settings, and select Save Changes. Field Type offers Text, Textarea, Number, Select Dropdown, Radio Buttons, Checkboxes, Date, and URL. A select, radio, or checkbox field needs at least one value in Options (one per line), and Required field makes the client complete it.A form must have at least one field, and each custom field must have a label.

  • Logo & Header sets Form Title Text (an empty value hides the title) and Select Logo, which opens the broker brand assets. A selected logo adds Logo Position, Width (px), and Height (px).
  • Color Scheme sets Primary Color, Background, Card Background, Text Color, and Accent Color. Background and Card Background can be transparent. Import Brand Colors copies the saved broker brand colors, and Back to Default restores the default form colors.
  • Form Customization groups Theme Preset (Compact, Default, Spacious), Typography, Spacing (Form Padding (px) and Field Gap (px)), and Layout (Width, Border Radius, Shadow).
Each added language has its own form title, description, success message, and active field text.
  1. Select a language in Add Language.
  2. Select Add Translation.
  3. Review the generated translation and edit the text when necessary.
Form Auto Translated generates the content again, and Remove deletes a translation. A translated form uses the selected locale in its direct link or embed code.
Staff members who enabled pending request notifications already receive notifications for manual form submissions. Use Additional Email Addresses (Optional) to add other recipients: enter an address and select Add.Use the delete control in Current Email Addresses to remove an extra recipient. The panel rejects an address that is already a staff notification address or already in this list.

Manage forms

Forms list with active service request forms Each row in Forms shows the form name, Direct URL, Approval, Status, creation date, and the Manage menu. Manage holds the sharing actions (Copy Form URL, Get Embed Code, Open Form), View Submissions (opens Requests filtered to the form), and the status, edit, and delete controls. The public URL controls are unavailable when the broker account has no slug; contact support if the page shows Broker slug not set. Please contact support. Edit a form: select Manage > Edit Form, change the settings, and select Update Form on the Notifications step. The changes apply to the public form. Activate or deactivate a form: select Manage > Activate or Deactivate. An Active form loads from its public URL and accepts submissions. An Inactive form does not load from its direct URL or existing embed code, and the submission service rejects its submissions. Delete a form: select Manage > Delete Form, review the message, and select Delete. The confirmation shows the submission count when the form has submissions.
Deleting a form cannot be undone. Deleting it does not delete its submissions; those requests remain in request history.

Form settings

Form Settings holds the broker-wide Trusted IPs and Manage Embed Domains controls, which only an Admin can change. See Publish and Embed Forms for their setup, rate-limit, and security details.
